Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Must have graduated from an American Physical Therapy Association (APTA) accredited school of Physical Therapy or equivalent. Possess a current Physical Therapy license within the State of Arizona. BLS certification is required. Demonstrated clinical competency, experience, training, and education relevant to the role.
- Additional Information: Position involves providing skilled physical therapy in a home health setting, assessing patient functional abilities, and developing individualized treatment plans. Responsibilities include documentation, interdisciplinary communication, patient education, and supervision of support personnel as applicable.
